VALIDITY EXTENSION for a valid U.S. Passport

If your passport was issued less than one year ago and is in
good shape, please gather the following items:

1--> Completed Passport Application:   C lick here for Form DS-5504 .
Fill it online, print it and sign it.
2--> Applicant’s current limited US Passport.
If this passport is damaged see New Passport Instructions and information.
3--> Proof of Applicant’s departure date from USA, such as:
I. Copy of Applicant’s actual airline or cruise ticket; or
II. Travel itinerary from travel agent or airline. FAX of the itinerary is generally acceptable; or
III. A letter (on company letterhead) giving the applicant’s name, date of departure, and
    the to which you are going. Address this letter “TO WHOM IT MAY CONCERN”.
4--> Two recent passport photos: 2”x 2”, frontal view, white background.
5--> If you received a limited validity passport to replace a lost or misplaced passport,
write a statement indicating you have not found your lost or misplaced passport.
If you have recovered the passport, you must submit it instead of this statement.
6--> U.S. Passport Expedite Application Fee: $60.00 + Our Service Charge
7--> RELEASE LETTER   Please call 1-877-727-7767 so that we may help you
complete this form correctly. You will need to complete the form, print it and sign it.
8--> Proof of applicant’s citizenship, such as:
I. Any previous passport in your name, even if it has expired but is in good shape; or
II. Original or certified copy of applicant's birth certificate issued by city, county, or state where
   you were born. It must have file number, date filed or registered (should be close to
   birth date), signature of issuing officer, and embossed or multicolored seal; or
III. Your original Naturalization Certificate.
