NAME CHANGE on your valid U.S. Passport
If your passport was issued less than one year ago and is in good
condition, please gather the following items:
1--> Passport Application: Please click here for Form DS-5504
Fill in the form online, print it and sign it.
2--> Applicant’s most recent valid US Passport.
If this Passport is damaged see New Passport Instructions and information.
3--> Applicant's documented proof of legal name change, such as marriage license,
divorce decree, court order, etc. This must be the original document or a certified
copy. The document will be returned with the passport.
4--> Two recent passport photos: 2" x 2", frontal view, white background
5--> U.S. Passport Expedite Application Fee: $60.00 + Our Service Charge
6--> RELEASE LETTER Please call 1-877-727-7767 so that we may help you
complete this form correctly. You will need to complete the document,
print it and sign it.
7--> Proof of Applicant’s departure date from USA, such as:
I. Copy of Applicant’s actual airline or cruise ticket; or
II. Travel itinerary from travel agent or airline. FAX of the itinerary is generally acceptable; or
III. A letter (on company letterhead) giving the applicant’s name, date of departure, and
the to which you are going. Address this letter “TO WHOM IT MAY CONCERN”.

Fill in the form completely, print it out, and sign it.
2--> Two recent passport photos: 2” x 2”, frontal view, white background.
3--> Applicant’s most recent US Passport.

6--> Proof of applicant’s departure date from USA, such as:
