MAKING CORRECTIONS on your valid U.S. Passport

If your passport was issued less than one year ago and is in
good shape, please gather the following items:

1--> Passport Application:   C lick here for Form DS-5504.
Fill it online, print it and sign it.
2--> Applicant’s most recent valid US Passport that has incorrect information.
If this Passport is damaged see New Passport Instructions and information
3--> Two recent passport photos: 2”x 2”, frontal view, white background.
4--> Applicant's documented proof of correct information, such as previous passport,
birth certificate, naturalization certificate, etc. This must be the original
document or a certified copy. The document will be returned with the passport.
5--> U.S. Passport Expedite Application Fee: $60.00 + Our Service Charge
Please note: The $60.00 fee may be waived, if the desired correction is due to an error
by the Passport Services Agency.
6--> RELEASE LETTER   Please call 1-877-727-7767 so that we may help you
complete this form correctly. You will need to complete the form, print it and sign it.
7--> Proof of Applicant’s departure date from USA, such as:
I. Copy of Applicant’s actual airline or cruise ticket; or
II. Travel itinerary from travel agent or airline. FAX of the itinerary is generally acceptable; or
III. A letter (on company letterhead) giving the applicant’s name, date of departure, and
       the country to which you are going. Address this letter “TO WHOM IT MAY CONCERN”.

Fill the form in completely and follow the instructions to print. Sign the form.
2--> Two recent passport photos: 2”x 2”, frontal view, white background.
3--> Applicant’s most recent passport that has incorrect information.

4--> U.S. Passport Expedite Application Fee: $135.00 + Our Service Charge
Please note: A portion, $75.00, of the application fee may be waived if the desired
  correction is due to an error by the Passport Services Agency

1--> Package all above 7 requirements for the appropriate set of instructions and
call 1-877-727-7767 to get the appropriate shipping address.

important to avoid delays in processing. Be sure to hit the SUBMIT button
