NEW ISSUE U.S. Passport for a CHILD (under age 18)
Please gather the following items:
1--> Completed Passport Application Click here to fill out Form DS-11 online.
Fill in the form completely, make a printed copy, but DO NOT SIGN the form.
2--> Two recent passport photos: 2”x 2”, frontal view, white background.
3--> One of the following as proof of US Citizenship:
Applicant's most recent passport and birth certificate (in English)
Applicant's US birth certificate (only if the applicant did not have any US passport)
Applicant's original Naturalization/Citizenship certificate
US passport (or Citizenship Certificate) of both parents and applicant's birth certificate
(translated to English)
4--> U.S. Passport Application Fee in check or Money Orders only:
Under 16: $120.00 + $25.00
Over 16: $135.00 + $25.00
5--> 2 Copies of Proof of Applicant’s departure date from USA, such as:
I. Copy of applicant’s actual airline or cruise ticket; or
II. Travel itinerary from travel agent or airline. FAX of the itinerary is generally acceptable; or
III. A letter (on company letterhead) giving applicant’s name, date of departure, and the
country to which you are going. Address this letter “TO WHOM IT MAY CONCERN”.
6--> 2 Copies of RELEASE LETTER. Enclose one copy “Inside the SEALED
ENVELOPE” and the other copy “Outside the SEALED ENVELOPE”.
7--> Proof of parents identity, (both mother and father) which has your picture
and was NOT issued recently, such as: State or Federal ID card, drivers
license, military ID, foreign passport, etc., as follows:
Applicant less than 16 years old: ID of both mother and father as shown on birth certificate
Applicant 16 to 18 years old: ID of either mother or father

PROCEDURE
1--> Applicant and parents (as explained below) are to go to a Passport Application
Acceptance Location (PAAL) with all of the above specified items.

Applicant is less than 16 years old:
Both parents and applicant are required to appear at the PAAL office,
or one parent can give a written, notarized authorization to the other parent
with a copy of drivers license or other ID.

Applicant is 14 or 15 years old:
See above for applicants under 16 years old.
Applicant 16 to 18 years old:
One parent is required to appear with applicant at the PAAL office;
or if the applicant has proper ID, either parent can give a written authorization
for the applicant to get a passport.
2--> Request the PAAL Officer to do the local Processing and Verification of your
application and advise the PAAL Officer that PASSPORT AND VISA EXPRESS will
hand carry your application documents to the US State Department Passport Agency.
3--> Request that all the appropriate items be placed in a SEALED ENVELOPE, and this
envelope be given to you.
NOTE: PAAL officer will give you the SEALED ENVELOPE. If there is any reluctance,
the PAAL officer should consult the “Hand-Carrying of Executed Applications”
section in the official “Passport Agent’s Manual” or call us for assistance.
4--> Write the applicant’s name, date of birth, and departure date on the SEALED
ENVELOPE and STAPLE one copy of the Proof of Departure Date on the outside of the
SEALED ENVELOPE.
5--> Call 1-877-727-7767 to get the appropriate shipping address for your SEALED
ENVELOPE. (We will advise you of the appropriate location when you call.)

6--> Send your SEALED ENVELOPE along with a personal or business check or Money
Order for Our Service Charges via FED EX.
(Please only use FED EX, and if that is not convenient, then call us for alternate instructions.)
7--> Complete the Work Order on-line form. Completion of this form is very important to
avoid delays in processing. Be sure to hit the SUBMIT button at the bottom of the page
when you have filled in the form completely.
