Please gather the following items:

Print it out and sign it.

2--> Applicant’s most recent valid US Passport.
  (if this passport is damaged see New P assport Instructions and information)

3--> U.S. Passport Expedite Application Fee: $60.00 + Our Service Charge

4--> RELEASE LETTER   Please call 1-877-727-7767 so that we may help you complete
  this form correctly. You will need to complete the form, print it and sign it.

5--> Proof of applicant’s departure date from USA, such as:
          I. Copy of applicant’s actual airline or cruise ticket; or
          II. Travel itinerary from travel agent or airline. FAX of the itinerary is generally acceptable; or
          III. A letter (on company letterhead) giving the applicant’s name, date of departure, and the
               country to which you are going. Address this letter “TO WHOM IT MAY CONCERN”.
