4--> RELEASE LETTER   Please call 1-877-727-7767 so that we may help you complete this form correctly.  You will need to complete the form, print it and sign it.

5--> Proof of applicant’s departure date from USA, such as:

I.  Copy of Applicant’s actual airline or cruise ticket; or
II. Travel itinerary from travel agent or airline. FAX of the itinerary is generally acceptable; or
III. A letter on company letterhead giving applicant’s name, date of departure, and the country you are going to.          Address this letter “TO WHOM IT MAY CONCERN”.
